AndreyandClaude Opus 4.8 059b488592 fix(conversations): AI failure hands dialog to operator instead of dropping it ...

On a persistent ProviderError the inbound message was already marked
processed (InboxEvent) and the messenger offset advanced, so a silent
return left the customer with no reply and no operator follow-up.

Now an AI failure: pauses the dialog to the operator queue, posts a
SYSTEM trace + a graceful customer-facing fallback, notifies operators
(DIALOG_WAITING, dedup aifail:<id>), and delivers the fallback via the
transport. No customer message is lost.
