Claude dbb83caa13 fix(studio): keep the assistant tool registry lists in sync ...

`filterToolsByOptInLevel` drops any tool that has no `TOOL_CATEGORY_MAP`
entry rather than erroring, so a tool registered in only one of the two
lists disappears from the assistant with no failing build and no log line.

`get_active_incidents` was categorized but missing from the
`toolSetValidationSchema` enum. That enum is currently only applied to MCP
tools, so nothing was broken today — but it is meant to be the registry of
legal tool names, and the gap would bite the first time the schema is
applied more widely.

Add the missing name and assert both directions in a test, so adding a tool
to one list and not the other fails there instead of in production.
